Our office prides itself on understanding your insurance in order to maximize your benefits while reducing your out-of-pocket expenses as much as possible. However, our financial agreement is between the office and the patient, not the insurance company. While our office strives to present you with a patient-centered and cost-effective treatment plan, we, at no time, guarantee how your insurance will process your claim. All claims are subject to final review by your insurance company. 

The estimated patient responsibility is due at the time services are rendered. Any service that is submitted to insurance and subsequently not paid by them, would be due within 30 days of date of treatment or prior to the beginning of the next phase of treatment, whichever is sooner.
